A 64 years old male patient barber by occupation came to casuality with chief complaints of shortness of breath and pain in abdomen  History of presenting illness:: Patient was apparently asymptomatic 1 month back and then he developed shortness of breath incidious onset and gradually progressive  No c/o chest pain,Palpations C/o fever for 3-4days ,high grade, evening rise of temperature associated with chills and rigor Since one month patient complaints of breathlessness (grade 3) insidious in onset gradually progressive aggrevated on walking and no seasonal variation.  Past History:: Decreased urine output since 6 months  c/o pain abdomen on left lumbar region since 1 week  patient is a known case of HTN since 10 years for which he is on medication (olmosetron tab) Not a known case of DM,TB,ASTHMA  Personal history: Apettite-decreased Diet- mixed Bladder- decreased Bowel -abnormal ##Addictions-  Tobacco chewing-stopped 3 yrs back Alchol-stopped 1yr ago Family history:  No significan

A 75 year old male came to opd with pain and swelling in left lower limb

A 75 year old male, resident of narketpally , farmer by occupation came to hospital on 19/9/22 with  CHIEF COMPLAINTS        Swelling and pain in left ankle since 1 week HISTORY OF PRESENT ILLNESS       The patient was apparently assymptomatic 7 days back.The he developed pain in the left ankle for which  he applied zandu balm  for  which he developed swelling  over the applied area . He has pain over that area which was gradual in onset , pricking type which was continuous and radiating from ankle to groin and was relieved on medication. He also had fever that was relieved on medication. He also has burning micturition since 1 week. It was not associated with any weight loss. PAST HISTORY       He had diabetes type 2 since 10 years        No history of HTN        No history of CAD , Asthma , epilepsy, TB        No past surgeries FAMILY HISTORY         Insignificant  PERSONAL HISTORY        Married        Farmer by occupation        Mixed diet         Decreased appetite        Adequate

Hi, I am Sathwika Reddy of 3rd semester .This is an online E logbook to discuss our patient’s de-identified health data shared after taking his/her/guardian’s consent. This also reflects our patient centered online learning portfolio. The patient’s consent was taken verbally prior to history taking and examination of her condition. CHIEF COMPLAINT: A 27year female patient who is a worker came to the casualty on 15thof July with complaints of high fever with chills and productive cough. HISTORY OF PRESENT ILLNESS: The patient had a fever spike so came for consultation.she again had a fever spike that is resolved spontaneous then she had no fever spikes. She underwent hemogram on 10 august no fever spike from 10 august. HISTORY OF PAST ILLNESS: No history of similar complaints in the past.
